Already have a vet
With your authorization we request your pet’s records from your previous clinic and handle the paperwork.
What moved to the visit rather than disappearing: street address, second phone, microchip numbers, and the authorized healthcare decision-maker — that last one is a third party’s name and phone, and it is better asked for in person than collected from someone who has not been told. Nothing here asks you to sign anything; this is intake, not consent.

Signing before a procedure
Surgery and sedation consent can be signed either way — at home before the day, or on a tablet at drop-off.
