Transport

Bring your pet in a secure, escape-proof container, warm in winter and out of direct sun in summer. Call us first if you are not sure.

The current version of this form asks thirty-four questions as thirty-four separate boxes, most of them a single line. This one keeps every clinical question and groups them the way a veterinarian asks them — heat with the thermometer that measured it, light with the bulb that produces it — so the answers arrive as a picture of the enclosure rather than as a list.
