What we collect, why we collect it, and who else sees it. This notice covers both of our websites — the two hospitals are owned and operated by the same two veterinarians.

What we collect, and why
When you fill in the new client form, we ask for your name, email address, mobile number, zip code, and your pet's name, species, breed, age and sex. We use it to open your record and to call you back and book your first visit. That is the whole purpose.
When you book online, request a prescription, or contact us through the site, the information you enter goes to the systems described below.
Like most websites, ours records basic technical information automatically — the pages you visit, roughly where you are, and what browser you use. We use it to see which pages are useful and which are broken.
Who else receives it
- PetDesk — our appointment booking and client communication provider. Booking and reminder information passes through their systems. TKDoes PetDesk receive new-client form submissions, or only booking data?
- Our online pharmacy — orders placed at the online pharmacy, along with your payment details, are handled by its operator rather than by us. TKThe legal name of the company operating the ourvet.com storefront. It has to be named here.
- Google — we use Google Analytics and Google Tag Manager to understand how the site is used. TKIs Google Ads, or any remarketing or conversion tag, running on either site? The contents of GTM container GTM-5TSKQQF decide this answer.
- Our email provider — sends the confirmation emails you receive from us. TKWhich provider. SPF lists both Google and Amazon SES, so there are probably two senders — Workspace for staff mail and something automated for the rest.
- Our practice management system — where your pet's medical record lives once you are a client.
We do not sell your information. We do not share it for anyone else's advertising.
Your pet's medical records
Medical and surgical records, including signed consent forms, are kept as part of your pet's clinical record and are retained for the period required of Arizona veterinary practices. TKFOR COUNSEL — this sentence must not ship until the retention period is settled in writing. It gates this page and the consent archive both. Do not build an auto-delete job against a guess.
Cookies
Our site uses cookies for basic function and for the analytics described above. You can block or delete them in your browser; the booking widget may not work if you do.
Children
This site is not directed at children and we do not knowingly collect information from them.
Asking us about your information
Call us and ask, or email us. TKWhich email address should take privacy questions? It needs naming here. We will tell you what we hold about you and correct anything wrong. If you want us to delete information that is not part of a medical record we are required to keep, say so and we will.
Changes
If we change how we handle your information, we will update this page and change the date at the top.

Preview note: this draft was written to be short, specific and true rather than boilerplate. It has not been read by counsel. The flags above are the six factual gaps plus the one clause held for legal review — clearing the flags is not the same as answering the questions.
