Bring, if you can
- A photo of the enclosure as it normally is
- The food packaging, or a photo of the label
- Droppings from the last day on clean paper
Exotic pets here
The current version of this form asks seventy-one questions, almost all of them as separate boxes, and marks a scatter of them required for no clinical reason. This one groups the husbandry questions the way a veterinarian actually asks them and requires only what an exotics appointment genuinely cannot start without. Anything you leave blank gets asked in the room.
