Surgery & sedation consent
This is the document you will be asked to sign before an anaesthetic procedure. Read it here, at your own pace. You do not sign it on this page — we email a signing link to the address on your record, or you sign it in person at drop-off.
How signing works
Two ways, one document
Whichever you choose, it is the same paper and a copy lands in your pet’s record.
A signing link, emailed to you
The link goes to the address already on your record and works once. That is what ties the signature to you, rather than to a name typed by whoever happened to open a public web page.
On a tablet, here
If you would rather read it with someone who can answer questions, sign it at the front desk on the morning of the procedure. Allow a few extra minutes.
Why not a sign-here box on this page: a public form can be filled in by anyone, for any animal, under any name. Twenty-two required acknowledgments signed by an unidentified person is a weaker record than no signature at all, because it looks like one. The emailed link fixes the identity, which is the part that was actually missing.
What you are agreeing to
Each item below is one of the acknowledgments on the consent document. They are set out here in full so that you can read them before the morning of the procedure, not while standing at a counter with a carrier in your hand.

Costs after the procedure

Same reasoning as the clause above: both hospitals are shown the union of the two papers until counsel rules on which set is correct. It is also a statement of fact about staffing that each hospital has to be able to stand behind.
Before the day
- No food after 10 PM the night before. Water is fine overnight unless we have told you otherwise.
- Tell us about every medication and supplement, including the ones that seem too minor to mention.
- Give us a number you will actually answer during the procedure. We call if anything changes.
- You will have a written estimate before anything begins. If the plan changes, we call you first.
Questions before you sign
Call Animal Hospital at Grayhawk on 480-585-3512 and ask for the surgery team. Nobody here would rather you signed something you had questions about.
Before the procedure
The details we need in advance
This is not the consent form and nothing here is a signature. It is the practical information the surgery team wants in front of them the day before — so that the only thing left on the morning is the document above.
On the morning
- No food after 10 PM the night before
- Bring the medications, in their containers
- Allow a few minutes at drop-off
